Community Connector
We have been chosen to be a Pilot Program from the Governor’s Cabinet on Aging! The grant provides us money for services in our community.
The funding allows us to provide:
- A Monthly Senior Friendly Events Calendar (Let us know if you have an event that you would like listed)
- The opportunity to bring more services to the area such as:
- Agencies on Aging
- AARP Tax Prep
- One-on-One Benefit Checks
- GU Transporters
- Medicare Counceing
- Money Minders
- Senior Companions
SACOPEE VALLEY, MAINE—In the heart of far western Maine’s Sacopee Valley, a new
pilot program is breaking down barriers and strengthening community ties for older
adults. The Grateful Undead Community Connector project is tackling long-standing
challenges in this fiercely independent region—where aging-related services are often
located more than an hour away and social isolation is a growing concern.
Launched across five towns and three counties, the Community Connector pilot meets
people where they are, ensuring they stay connected, informed, and supported in
meaningful ways.
A Vision Grounded in Community
The pilot is guided by three key goals:
Combating loneliness and isolation—recognized by the Surgeon General and
World Health Organization as public health risks comparable to smoking.
Bringing aging services closer to home through partnerships with local
organizations and statewide groups like AARP, agencies on aging, and
community action programs.
Offering personalized benefit checks and navigation support, helping
individuals access programs, resources, and events tailored to their needs.
Putting Faces to the Mission
At the heart of this effort is Jamie Gleason, a local outreach worker and long-time
resident, who serves as the program’s Community Connector. Jamie’s role extends far
beyond referrals: she meets face-to-face with individuals to explain options, help
complete applications, and stay engaged in both services and community life.
A Calendar that Builds Belonging
A standout success of the project is the Senior Friendly Events Calendar, a monthly full-
page feature in the Your Weekly Shoppers Guide newspaper, which reaches 13,500
households. It lists social, recreational, and educational events welcoming older
adults—many of whom previously felt disconnected.
“Seeing everything in one place helps me find exactly what I need,” shared one
community member.
In April alone, the calendar featured 58 events, attracting more than 400
attendees—many of whom joined multiple gatherings. One resident, previously battling
isolation and depression, now attends events regularly and even offers rides to others.
“He told me it’s getting him out of the house and giving him a sense of purpose,” Jamie
shared. “He said he’s starting to feel like the person he used to be. It was powerful to
see him smile.”
Bringing Services Directly to Residents
Jamie has also worked to bring essential service providers directly to the community.
For example, the Southern Maine Agency on Aging (SMAA) now offers one-on-one
appointments at a local town office.
A notable partnership with AARP Tax-Aide and the Sacopee Valley Health Center
resulted in three local tax clinics this year. Thirty-seven individuals received assistance,
securing $29,543 in refunds—money that often goes straight to necessities like property
taxes. The success of these clinics means the area will become an official AARP Tax-
Aide site next year, making this critical service more accessible for everyone.
Personalized, One-on-One Support
Beyond events and programs, the core strength of the Community Connector model lies
in its highly personalized support. Jamie makes over 30 individual connections every
month, helping residents navigate Medicare Savings Program applications, SNAP
enrollment, fuel assistance, and transportation options. These sessions often last more
than an hour and include follow-ups, ensuring residents aren’t left to navigate complex
systems alone.
Town offices have become vital allies in this effort, providing private meeting spaces and
referring residents directly to Jamie. Local libraries, the Sacopee Valley Health Center,
and Adult Education programs also collaborate, hosting workshops and extending the
project’s reach.
Looking to the Future
The early impact of the Community Connector pilot is undeniable—reducing isolation,
improving access to services, and strengthening the social fabric of Sacopee Valley.
The Grateful Undead team is already looking ahead to a second year of the project, with
plans to streamline volunteer training between SMAA and SeniorsPlus. The vision is
clear: a community-centered model where neighbors support neighbors in lasting, life-
enhancing ways.
Dementia Friends is a global movement that is changing the way people think, act, and talk about dementia. Developed by the Alzheimer’s Society in the United Kingdom, Dementia Friends helps everyone in a community understand what dementia is, how it affects people, and how each of us can make a difference.
